All recipients of our items have been referred to us by other Agencies or Organizations such as the Department of Social Services, Minnehaha County Human Services, Center of Hope, The Community Outreach, Lutheran Social Services, Children's Inn, Churches, Schools and Health Care Providers.

If you are a member of one of these agencies and you are aware of someone in the community who is low income and in need of baby items, encouraging words to attend healthcare appointments, please complete this referral form. If you do not know the password, please give us a call at (605) 335-2730 .

Referral Process Steps for Referring Agencies:
  1. Complete Online Referral Form - if you do not know the password for the referral form, please call (605) 335-2730 . Passwords should not be shared.
  2. Clients must sign the The Helpline Network of Care . All adults need to sign a separate form. If an adult in the household is not present, please indicate on your referral form. The Teddy Bear Den will follow up with the adult for consent. After you have signed the consent form, please email to referrals@teddybearden.org. (Click the “Get Started” and enter the password to start this process).
  3. Please remind clients they need to follow up with The Teddy Bear Den in 2 to 3 days to check the status of the referrals. Once approved, the referral stays open for 6 weeks.

In order to receive items from The Teddy Bear Den, recipient must earn points through our program. All referrals are treated with the utmost discretion.
